You are a GP, and your first patient of the day is 33 year old Isabelle Jones. She is worried about a mole on the skin of her forearm, which has been getting bigger recently.

You tell Isabelle that she may have an atypical mole (dysplastic nevi) but often they can resemble melanoma, and you want her to have further investigations with a dermatologist as soon as possible.
A fortnight later Isabelle is back to see you as you have the report form the dermatologist. You explain to Isabelle that the mole is indeed a tumour and that it will be important to determine whether the tumour has spread to the lymph nodes.
The diagram below depicts a typical lymph node.

Isabelle is still wondering how this could have happened. “Could this have been picked up earlier, before it became a cancer?” she asks. You explain that although there are screening programmes for cancer in Scotland, there isn’t one for skin cancer.
